Program Synopsis

AIDS Healthcare Foundation established Positive Healthcare in 1995 to develop managed care programs that really work for people living with HIV. In 1999, the State of Florida's Agency for Health Care Administration contracted with AIDS Healthcare Foundation to set up an HIV disease management program for MediPass patients with a diagnosis of HIV or AIDS.
This disease management program has been highly successful. Over 9,000 MediPass recipients have joined Positive Healthcare. There are six Positive Healthcare regional offices across Florida. Our staff has grown to include a Bureau Chief, 2 Physician Advisors, over 100 Registered Nurses and LPN Care Partners, all of whom specialize in HIV medical care.
Our goal is to work as a collaborative practice model to include the physician, support-service providers and the RN Care Managers to make the outcomes of the patients with HIV/AIDS in Florida as good as they can be. Here’s how we do it:
Expert Educational Opportunities and Consultations
The field of HIV medical care evolves quickly as new research findings and medications become available. It can be a real challenge for health care providers to keep up to date, particularly if the practice doesn't have a large number of HIV-positive patients.
Positive Healthcare utilizes treatment protocols based upon the best practice models of AIDS Healthcare Foundation, the Centers for Disease Control and Prevention, Department of Health and Human Services and other respected medical organizations. We always take into account the needs and expectations of African Americans, Hispanics, Haitians, and other groups making up Florida's HIV population.
We share this information with the provider at our many accredited CME/CEU seminars. Our physician advisors are also available as professional consultants.
Patient Education Program
Core to Positive Healthcare’s success is our program of in-depth and ongoing patient education and advocacy. Our Registered Nurse Care Managers keep in regular contact with their via one on one contact and the distribution of up-to-date materials covering a broad range of HIV-related and other medical topics. They are distributed to the patients and providers on a regular basis.
24-Hour Nurse On-Call Line
Positive Healthcare’s Nurse On-Call Line is available to answer your patient’s questions about their HIV, nutrition, exercise, disease management plan of care, questions concerning accessing medical care, and, in the case of an emergency, direct them to obtain the emergency care they may need. This Nurse On-Call Line is staffed after hours, weekends and holidays by our team of experienced RNs.
Effective Medical Data
We collect a vast amount of HIV medical data using our proprietary PosiTrak database. PosiTrak includes a clinical history and claims profile for all of the patients in the program, including office visits, lab results, medications, inpatient and outpatient utilization of services, pharmacy prescription filling information, and other support-health system usage. We provide utilization review information to the State and the providers, as well as trend analysis for our required reports. Upon the request of the provider, we can compile this data. Most providers find this information invaluable in the management of their patients.
A Helping Hand for You
Our RN Care Managers are here to assist the providers and patients with their HIV plan of care. We offer a full spectrum of disease management services. These services range from assisting the patients in making appointments; to helping the patients implement the plan of care developed by both the provider and the patient. The RN Care Manager provides one-on-one follow up and education in the privacy of the patient’s home. It's like having a helping hand outside the office, reaching out beyond the providers doors to help assure adherence to the plan of care.
